RPM can benefit a wide range of patients. In any
situation where more data and monitoring would
help you to treat an illness or condition, RPM is a
great fit. There are three types of patients that it
is especially beneficial to:

● Patients over the age of 50
● Patients with multiple conditions
1. First, your patient will be referred for RPM by their provider. This can happen for a variety of reasons, but generally happens when the patient is living with a condition that would benefit from daily monitoring.
2. Next, insurance coverage is discussed. We will find the level of coverage that each patient has so that RPM can remain a beneficial treatment option, rather than becoming a financial burden.
3. After insurance is figured out, the patient will be sent their RPM consent forms. Along with providing their consent to be monitored on daily basis, these forms will also help them identify their goals for their RPM experience.
4. With goals and consent secured, the patient is set up with a monitoring device. This will be decided with their provider and it will be based on the type of information the provider wants to monitor, as well as the specific conditions the patient is living with.
5. Finally, they simply use the monitoring device. They are encouraged to interact with the health coaches, especially in the critical first month. Health coaches will be checking in with the patient at regular intervals. Approximately 40 minutes of interaction time throughout the month is typical.
